聚賢樓入住申請表（澳門大學校友專用） UM Guest House Application Form (For UM Alumni)
 
  • 本申請表只作申請標準雙床房。
  • 申請人須為澳門大學校友。
  • 客房只可給予已作出登記之申請人及其親屬入住，房間不可出讓或轉交至第三方。
  • 住宿費用在入住時於聚賢樓接待處繳付，而已繳交之費用將不設退款。
  • 每位校友最多只允許申請兩間標準雙床房入住，每間標準雙床房供兩位客人入住。
  • 申請人須於客人入住前至少7個工作日提交申請表。
  • 如預訂有任何更改或取消，申請人須至少於客人入住前3個工作天以電郵通知學生資源處，否則會被收取等同一晚房費之特殊費用。
  • 申請人須親臨聚賢樓前台出示澳大校友卡/身份證明文件，並陪同親屬辦理入住。

收集個人資料聲明 澳門大學根據第1/2006號法律作為一所公立高等教育機構，將處理在本表格內所收集的個人資料作舉辦活動、提供服務及聯絡之用。 基於活動或服務所需，上述個人資料有可能轉交本澳相關機構。申請人有權依法申請查閱、更正或更新其存於本校的個人資料。 (如透過網上收集)透過電子網絡傳送的個人資料有可能被未經許可的第三者盜用。

  • This application form is for Standard Twin Room only.
  • The applicant must be alumni of the University of Macau.
  • Guest rooms are solely for the use by the applicant and guests registered and cannot be transferred to any other party.​
  • Payment shall be made at the reception upon check-in. Any payment that has been made for the accommodation fees is non-refundable.
  • Each alumni is eligible to apply for a maximum of two Standard Twin Rooms, each room can accommodate up to two guests.
  • The applicant shall submit application form at least 7 working days prior to the guest's check-in date.
  • The applicant shall notify Student Resources Section of any amendment / cancellation of the application by email at least 3 working days prior to the guest's check-in date. Otherwise, a special charge of 1 night room rate will be applied.
  • The applicant shall present UM alumni card or personal ID and check-in with family members at UM Guest House reception in person.​​

Personal Data Collection Statement The University of Macau being a public institution of higher education as set in Law No. 1/ 2006, will process the personal data collected on this form for activity organization, service providing and contact purposes. Due to the needs of the activity or service, the personal data on this form may be transferred to other organizations in Macao. The applicants have the right to access, rectify or update their personal data stored at UM. (if collected online)The personal data circulated on the network may be at risk of being used by unauthorized third parties.
